The Discharge Planning Department at St. Christopher’s Hospital for Children is staffed with registered nurses experienced in pediatric care. As part of the healthcare team, the discharge planners work closely with physicians, staff nurses, social workers and all other members of the patient care team.

During hospitalization, the healthcare team identifies any potential needs after discharge. The attending physician will direct the team to make the necessary referrals for services in coordination with the patient and family’s needs and preferences.

Services include: 

• Durable medical equipment and supplies
• Home nursing visits
• Home IV infusion
• Referrals for inpatient rehabilitation
• Referrals for inpatient skilled nursing placement
