Center for Child and Adolescent Health
The Center for Child and Adolescent Health at St. Christopher’s Hospital for Children provides comprehensive primary care for infants, children and adolescents encompassing both routine health services and the coordination of care for children with chronic illnesses and complex diagnoses. Through a network of pediatric centers in the community, we also provide convenient access to health services for children in the Greater Philadelphia area.

Our outpatient care for infants, children and adolescents includes evaluation and management of acute illnesses, immunization, lead screening, routine well-baby visits and other services.

Medical Home Model
The Center for Child and Adolescent Health at St. Christopher’s Hospital for Children is a medical home that brings all pediatric and adolescent primary care under one umbrella. St. Christopher’s program has an emphasis on pediatric patients with chronic illnesses such as asthma, obesity, mental and behavioral health issues and poor oral health. Studies have shown that medical homes provide improved health-related outcomes for children with special health care needs.

St. Christopher’s primary care patients will gain enhanced access to care through open scheduling, expanded hours and new options for communication between patients, their personal physician and practice staff. A care coordinator will be assigned to all patients. In addition, families will play a more active role in their child’s medical care.

In developing a medical home, St. Christopher’s is working to provide comprehensive healthcare for its patients, and respond to the specific needs of its community.
