Next Steps Program

The Next Steps Program (Neonatal Continuing Care Clinic) at St. Christopher’s Hospital for Children is one of the first Neonatal Intensive Care Unit (NICU) follow-up/primary care clinics in the region providing a continuum of care for infants that were cared for in the NICU.


Upon discharge from the Neonatal Intensive Care Unit, the Next Steps Program serves as a bridge until an infant is ready to be cared for by a general pediatrician.  The program offers a comprehensive and coordinated care model for up to two-and-a-half years post NICU discharge for an infant’s medical and health care needs.  This includes immunizations, well-baby visits and other primary care services.

The Next Steps Program utilizes a medical home that includes patient- and family-centered care, periodic screening, care coordination and medical services provided by a team of neonatologists, general pediatricians, pulmonologists, dietitians and social workers.

A Medical Home

A medical home is an approach to providing comprehensive primary care that facilitates partnership between patients, physicians and families. The American Academy of Pediatrics believes that every child deserves a medical home where care is accessible, continuous, comprehensive, patient- and family-centered, coordinated, compassionate and culturally effective.


All infants discharged from the Neonatal Intensive Care Unit whose families desire a coordinated care model in a medical home setting are eligible to participate in St. Christopher’s Next Steps Program.  Our staff will recommend an appropriate care plan customized to meet each newborn’s unique needs. The average transition to a general pediatrician is approximately three to six months.
